A lot of people are talking about it but at Summit Care, patient centered care has been a core part of our philosophy for years. It just makes sense.

In patient centered care, the role of the therapist becomes even more important. Because the caregivers are more familiar with the elderly, the therapist has more information available to them and can form a better plan of care. The therapist also brings his or her specialized expertise into the team around the elderly to elevate his or her level of physical function. By integrating therapists and other professional expertise into this environment, the outcomes are enhanced.

At Summit Care, we embrace these and other initiatives to improve the quality of service we provide and the level of participation we have from our residents. We are proud to say that we have created both long- and short-term living environments as well as community-based settings where both older adults and their caregivers are able to express choice and practice self-determination in meaningful ways at every level of everyday life.
